Adult Day Service
A support service which provides supervised activities in a group setting for the frail elderly as well as those with Alzheimer Disease and other progressive cognitive disorders or dementias of aging to assist them in achieving and maintaining their maximum level of functioning, to prevent premature and inappropriate institutionalization and to provide respite and support for their caregivers. Service components will include transportation (if required), meals, supervision, exercise, music, fellowship, activities, and minor health care (for example, monitoring of medication).

Eligibility Criteria
Persons with Alzheimer Disease or other progressive cognitive disorder or dementia OR Age 55 or over AND Long term physical disability including heart disease, stroke or seizures, arthritis, breathing difficulties or impairment due to medication. Priority will be given to clients who are unable to participate in other community support activities because of their current health status and to clients whose caregivers have been identified as requiring regular relief from care giving responsibilities.
Application Procedure
The Kingston, Frontenac, Lennox & Addington Community Care Access Centre (KFL&A CCAC) completes social, mental and functional assessments prior to determining if an individual is eligible for the Central Frontenac Community Services Corporation Adult Day Program.
Having determined eligibility, the KFL&A CCAC will make the referral and provide copies of the assessment to CFCSC.
Upon receipt of the referral and the assessments, the Long Term Care Services Coordinator will contact the potential participant to arrange a home visit to complete the application form and to provide information about the service.
The KFL&A CCAC will prioritize admittance to the Adult Day Service. In situations where immediate admission to the Adult Day Service will prevent premature or inappropriate institutionalization or is required because of a care-giving crisis, the Long Term Care Services Coordinator will make this home visit an immediate priority.
Eligibility Restrictions
Clients must be able to move about and use the toilet with minimal assistance and be able to take pre-packaged medication when prompted. Clients who require lifts or two-person transfers, suctioning, or dressing changes will not be admitted to the program.

Caregiver Support provides caregivers with emotional support, and educational information in order to help the caregiver make informed care decisions. Caregiver services are available to a wide range of caregivers, including: spouses, adult children, any family member caring for another family member, or any neighbour giving care support to another neighbour. The majority of caregivers ask for help in complex care planning, dealing with loss, and coping with the emotional burden of care. In decision-making, the care-receiver is included as an equal partner.

Service Parameters:
Caregiver support services are available to a husband caring for a wife, or wife caring for a husband. Service is also available for adult children caring for a parent, any family member caring for another family member, or a neighbour caring for a friend. Service is provided for caregivers supporting a care-receiver in the home, in a care facility, or in a hospital. Service is normally provided in the caregivers' home due to the rural nature of the catchment area.
Caregiver SUPPORT Services Provides:
- Information to assist the caregiver/care-receiver make the best care choices.
- Emotional support to give the caregiver the chance to express feelings of frustration, sadness, and anger.
- Advice on care planning based on the expressed needs of the caregiver and care-receiver.
- Educational information to help the caregiver better understand the disease process and care options.
- Assistance with the placement process into a care facility.
- Referrals to other agencies to increase service support to the caregiver and care-receiver.
- Educational workshops to help the caregiver/care-receiver maintain independence in the community.
Client Intervention and Assistance
Provides support and assistance to clients experiencing a sudden change in circumstances, may include preventing homelessness, financial support, food security, and liaison with more appropriate service providers.
Diners Club
Provides full-course, catered meals with the flexibility to meet varying dietary needs. For a cost of $11 per person, you will enjoy a wonderful mid-day meal, entertainment and transportation (if required). This is a great opportunity to meet new and old friends, enjoy some local talent, and learn what's up in our community.

Service Note:
- Diners Clubs are held from September - May.
- The first Thursday of the month is held at the Trinity United Church in Verona.
- The second Thursday of the month is held at Harrowsmith Free Methodist Church.
- The third Thursday of the month is held at Inverary United Church.
- The fourth Thursday of the month is held at Harrowsmith Free Methodist Church.
- Reservations for Diners are required by noon the Tuesday before the meal.
- Transportation may be arranged if needed. Please make the office aware at the time you register.

General Application Process for Financial Assistance Programs
Applicants must be residents of South, Central, or North Frontenac Township. Low-income families with children 0-18, low-income seniors, and physically disabled adults will be given equal consideration. Single adults and couples with no dependents will be considered based on income and situation. Applicants must have exhausted all other resources. Applicants must be living in a housing situation they consider affordable, permanent, and adequate.

Eligibility Criteria for Financial Assistance Programs
- Call the Rural VISIONS Centre and explain your need.
- Let us know where you live, your phone number, and your family composition.
- If you receive income from OW or ODSP you will be directed to approach them before proceeding further with any application.
- If all other resources have been approached, book an appointment.
- At the appointment you will fill out the most appropriate application form to address your need.
- Applicants must sign a release of information allowing the agency to correspond with other parties involved (i.e. landlord or utility company).

- Stub for Employment, EI, OW, ODSP, WSIB, CPP, OAS, etc.
- GST, CCTB, Child Support
- Bank statement for all accounts

- Utility Bills (Hydro, Propane, Oil, etc.)

Financial Assistance Programs
Caring Community Fund
(Maximum $400.00)
Community Caring is a charitable organization in Hartington that sells gently used clothing. They provide our organization with annual funding to offer financial assistance for heating, hydro, personal health expenses and groceries. This program is for one-time emergencies and not for recurring use. This program is available to residents of South Frontenac Township only.
Consolidated Homelessness Prevention-Programs
The Consolidated Homelessness Prevention Programs are designed to assist tenants and homeowners in maintaining their housing. These programs may be accessed once annually.
Housing Loss Prevention Assistance Program
(Maximum $1500.00)
Homeowners can apply for assistance with mortgage or property tax arrears.
Access To Affordable Housing Assistance Program
(Maximum $750.00)
Tenants can apply for assistance with either first or last months rent.
Energy Source Equipment Upgrades Program
(Maximum $1500.00)
Grants are available to assist with heating and utility upgrades such as oil tank or fuel pipe replacement. Proof of work required must be submitted with the application. If the work required exceeds $1000, up to three quotes may be requested.
Homelessness Prevention Program (replaces TAP)
(Maximum $750.00)
The Homelessness Prevention Program assists individuals in maintaining their housing when faced with loss of housing due to property tax arrears or in extenuating circumstances mortgage arrears. This program may be accessed once annually.
Share the Warmth
(Maximum $200.00)
Share the Warmth is a provincial organization that partners with local organizations between November and March. Grants are available to assist with hydro, oil and wood purchases. This program may be accessed once annually.
Rent Bank
(Maximum $ Two Months Arrears)
The Interim Rent Bank Program assists individuals in maintaining their housing when faced with the possibility of eviction due to rental arrears. This program is for one-time access and not for recurring use.
Utility Bank
(Maximum $750.00)
The Interim Utility Bank assists individuals in maintaining their energy source when faced with the possibility of disconnection or non-delivery due to arrears. This program may be accessed once annually.
Winter Warmth Fund
(Maximum $450.00)
Offered through United Way, the Winter Warmth Fund operates from December to April. It offers grants to low-income applicants faced with disconnection of their heating source due to arrears. This program may be accessed once annually.

A Nurse to counsel and care for feet of seniors or physically disabled adults at three monthly clinics throughout our area. Diabetic referrals are accepted. The Fee is $12 per session at clinics and $15 per session for home visits.

Clinics are held at the following locations:
- The first Friday of every other month at Country Pines Apartments in Glenburnie. Appointments start at 9:30 AM.
- The second Tuesday of every month at the Verona Medical Clinic in Verona. Appointments start at 9:00 AM.
- The second Tuesday of every month at the Rural VISIONS Centre in Sydenham. Appointments begin at 1:00 PM.
Foot care may be provided in home if the client is legitimately unable to get to a clinic.

This is a free service that provides social interaction, in person to those unable to get out much or that are new to the community.
Home Helpers
Provides assistance with routine housekeeping or physically challenging activities required to maintain your home. Our Staff arrange for screened, insured workers to complete the jobs you need done. Home Helpers can come weekly or bi-weekly. The cost for this service is $12 per hour.

- Approved activities include ~ vacumming, sweeping, mopping, washing floors, cleaning bathrooms, and kitchens, interior window washing, general housekeeping, dusting, assistance with laundry.
- Homehelpers cannot assist with bathing, dressing, foot care or hair care.
- Service will be discontinued if clients have not paid their invoices for three months without giving Staff a valid reason.
Meals on Wheels
Offers a choice of 31 menu items. You choose what you eat and when you eat it! The meals provide 40% of your daily nutritional requirements including special diet needs. These meals can be heated in your microwave or regular oven. The main course meals cost $4.50 each. You can pick up your meals or have them delivered monthly.

Respite Care
Provides increased coverage for Caregivers receiving assistance from the Community Care Access Centre or for relief of Caregivers when periodic breaks are needed. Health care trained providers offer periodic or ongoing respite. The cost is $12 per hour.

Approved activities include ~ meal or snack preparation, assistance with toileting, assistance with dressing, assistance with bedtime and waking activities, supervision, reminders to take medicine, activities that could occur as the result of a clients condition, companionship.

Transportation Service Notes:
Approved destinations include ~ medical appointments, day surgery or hospital visits, therapy appointments, veterinary appointments, hospital / LTC facility visits, train station, bus station, airport, government or law offices, banks, post offices, grocery shopping, hair appointments, holiday shopping.
- CFCSC reserves the right to book more than one client on trips.
- CFCSC reserves the right to cancel a trip if weather reports are not favorable.
- CFCSC will arrange escorts at a cost of $12 for clients requiring physical assistance or accompaniment.
- Clients are always responsible for parking fees.
In the instance that a client is attending an event, appointment, and/or social outing that will take more than four hours from start to completion; the volunteer driver may drop off the client and return for the pick-up when the event, appointment, and/or social event ends. In a case such as this, the driver may bill CFCSC for the cost of two trips. In these situations, CFCSC will bill the client for the cost recovery.
Service will be discontinued if clients have not paid their invoices for three months without giving Staff a valid reason.

Caring Friends Volunteer Support Network
Caring Friends are experienced community bereavement volunteers. Caring Friends provide support to the bereaved by offering practical advice on the grief process, and emotional support to help the bereaved cope with loss.
Caring Friends volunteers offer telephone reassurance, and in-home visits to the bereaved in this rural community.
